The T-shirt has a ribbed crew neck and short sleeves. The front of the T-shirt is printed in red, blue, white, and gray with the words "2004 NCAA National/ Champions/ University of Connecticut/ Huskies/ UConn Men's Basketball/ UConn Women's Basketball/ Home of the/ Champions." In the center of the image is a faux-marbled gray representation of the state of Connecticut draped with a red, white, and blue swag. On top of the state is a large white UConn husky logo. To the proper left of the husky is the UConn men's basketball logo, consisting of an orange basketball behind an intertwining white U and navy C. To the proper right of the husky is the UConn women's basketball logo, consisting of an orange basketball behind an italic navy C.

Satin-faced white ribbon with black and blue printing is sewn into the center back inside. The front says "L/ 99% Cotton Preshrunk/ 1% Polyester/ Made in Honduras" in Spanish, French, and English. The back gives washing instructions, printed in blue, in the same three languages and features the following numbers at the bottom, "RN 93846 CA 25181."
