There is a portrait of Israel Putnam on the front, within a pediment, flanked by the French and another flag with a red ground and blue and white x canton. Below the portrait there is a painted white ribbon with text that reads, "He dared to lead where any dared to follow." Below that is text that reads, "PUTNAM PHALANX."
On the reverse is a similar scene, with a central image (the Connecticut seal with three grapevines and the words "QUI TRANSTULIT SUST.") within a pediment, flanked again by flags. Below the seal there is a painted ribbon with text that reads, "The memory of PUTNAM is not forgotten." Below that is text that reads, "PUTNAM PHALANX."
