DescriptionMedal made of yellow metal, with text and images on both sides. The center of the front side is an image of a colonial-style building. The words "CONNECTICUT STATE BUILDING" curve around the top edge. The bottom of the medal is printed "SESQUI CENTENNIAL/ PHILADELPHIA/ 1776 [image of Liberty Bell] 1926".

The back of the medal depicts a statue on a pedestal on the left side, with a leafy branch below it, curving around the bottom of the medal. The statue is a young man dressed in colonial clothing, including a long coat and stockings. The upper right corner of the medal shows the crest of the state of Connecticut. Printed below the crest and to the right of the statue is, "'I ONLY REGRET THAT I HAVE/ BUT ONE LIFE TO LOSE FOR/ MY COUNTRY'/ NATHAN HALE/ PATRIOT-MARTYR/ CONNECTICUT/1755/1776".
